Summer 2025 Participant Feedback
We are seeking feedback regarding your Summer 2025 playing experience with UDFC.
The responses from this survey will be reviewed by our staff as we seek to improve our programs moving forward. Please fill out a separate survey for each program you are evaluating where appropriate. We invite every member of our club from all ages and levels to complete this survey.
Accessibility & Inclusion Survey
To help us better understand your experiences — especially regarding accessibility, inclusion, and any barriers you or others may have faced while participating in our programs — we invite you to complete a short, anonymous survey. Your feedback will play an important role in shaping how we improve our programs and policies moving forward.
UDFC Jersey FAQ
The FAQs about jerseys can be find HERE
JERSEY RETURN
When your player's season is done, please return their jersey(s) to the team manager or coach (whoever collected your deposits). Once this is done, you will receive your deposit back. We are not allowing players to keep their jerseys at this time as this is still a new system that we're rolling out and we need all of our jerseys back.
If you have lost a jersey, we need to know ASAP so we can have it replaced in time for the next team and season. PLEASE DO NOT RETURN YOUR JERSEY TO THE OFFICE unless you have pre arranged it with the manager.
The front desk staff are not involved in the jersey program and it will possibly create confusion and potential problems.
Please keep an eye on the Need to Know newsletters for updated information regarding how to acquire new jerseys and new gear.
